Students in grades 6, 8, and 11 will take part in the Oregon Student Health Survey during the window of October 6–17. The survey is anonymous and voluntary. Families may review the survey questions and opt-out form in the attached letter; if you do not want your student to participate, please return the form to your school’s office before October 6.
